Beyond Operator Error: A Fresh Look at Incident Analysis

Jeff’s presentation delves into the emerging systems-based approach to risk management, which has altered the way we conceive, organize, and implement our systems for safety. This workshop provides a new perspective that looks beneath staff error, and zeroes in on the root causes thereof - the internal organizational reasons for accidents. He explains how, in effect, we oftentimes set up our staff to fail, and how to avoid this. 

Jeff is a professor and coordinator of Algonquin College's Outdoor Adventure Program, where he manages 14,000 student field days per year. He has worked as a guide, educator and manager since 1988, crossing a variety of disciplines, activities and corporate cultures. His PhD work is in safety motivation and safety behavior, and he speaks extensively on risk-related topics. Jeff is the co-author of Managing Risk, Systems Planning for Outdoor Adventure Programs. His consulting work has taken him around the world for tourism development, staff training and risk management projects.
